how do you get the black h4 from zammy boss do you have to kill the boss or does it spawn when you enter the room

Kill zammy boss or his minions. They have a chance to drop blach h4. Keep killing them until you get it.

is anyone going to update this? ive got 14 achievements done(much appreciated) but i would like to finish all of them with more guidance if possible please
